Process for new patients at MDSMC

Our mission is to provide our patients with the highest level of care at convenient hours and an accessible location. We guarantee that all calls from new patients will be seen within 24 hours of contacting our office. We also offer evening and early morning physical therapy appointments to accommodate the busy work schedules and lifestyles of our patients.

When you call to make an appointment, our trained billing staff will take your insurance information and call on your benefits. Every patient will be contacted prior to their first appointment to discuss their policy’s benefits, deductibles and co-insurance.

If you are visiting us for the first time, please arrive 15 minutes early before your appointment to allow time for registration. The registration and patient history forms can be found below. Please bring your forms filled out along with your driver’s license and insurance cards.
